Can anyone tell if this is Photoshop or real? Some of the belts look a little weird and so does the duct tape.

At a quick glance it might be hard to see, but if you actually look at it for a few second's you can see the belts don't press on her skin normally nor does the tape

It seems very real to me, maybe the gag is the only thing that if you look closely it can be an addition of photosop.

That's photoshop all the way, the belts don't press into the skin, nor is there any bend like there should be. Just 2d images pasted in, unfortunately.
