Great story, managed to get all 4 points, not going to give out spoilers though!

Great story - best I've managed is having a sub in a leather sleepsack for 10 hours solid - chained and roped to a bed. He could move about 5mm each way.

One of the best I've read for a while - you really know how to build the reader up and leave then hanging for more.
